By submitting this AIDT transportation request form, the employee agrees to the following conditions of use: This is a limited transportation service provided for AIDT and its affiliates at no cost. Transportation services are made available at AIDT’s sole discretion on a case-by-case basis for eligible applicants within a 30-mile radius of the plant. Location, road conditions, and economic practicality will all be taken into consideration. Initial ride requests must be received 48 hours in advance. For any cancellation, passengers must inform AIDT 24 hours in advance of the scheduled pick-up/drop-off. The State of Alabama, AIDT, and any of its employees disclaim all liability for any damages, loss, or injury to person or property incurred during use of this service. AIDT reserves the right to suspend or cancel transportation services at any time and for any reason including but not limited to unsafe conditions, inappropriate conduct, employee termination, or failure to communicate (i.e., no call/no show). These services are subject to Alabama state law, and, in the event of dispute, AIDT’s dispute resolution policy shall govern.
